Office Visit BEFORE Colonoscopy with Dx of Personal History Of Colon Polyps

Someone (and I forget where I heard this) told me it is acceptable to use Z09 as primary dx code when a patient comes into office with dx of Personal History Of Colon Polyps before a colonoscopy in order to get the office visit paid for. So you would bill Z09 as primary code and Z86.010 as secondary code. Anyone know if this is correct?

Hx of Polyps – Here we go again

I know we go around and around on this, so…….
Patient was seen in 2010 with a history of polyps (unknown if tubular or hyperplastic). Also unknown when colon prior to 2010 was performed. Patient had the procedure in 2010 and nothing was found. Patient also has a family history of colon cancer. Patient comes back this year for a colon and nothing was found. Since its been over 10 years since polyps were found is this now considered G0105 due to family history or should we be using CPT 45378 w/ history of polyps diagnosis?
Would appreciate any input on this

Screening colonoscopy, multiple polyps

When performing a screening colonoscopy, the surgeon removed a polyp from the ascending colon with a snare (CPT 45385,33). He also removed another polyp in the ascending colon (3 cm. away from the other polyp) with hot biopsy (CPT 45384, 33).

Are both polyp removals billable? My thought is if they are in the same anatomical location (ascending colon), even though they were two separate polyps, they are not both billable.

Any resources would be greatly appreciated!

Two Polyps in Transverse Colon

Horizon BCBS has performed an audit on our procedures, and they stated that when two polyps are found in the same area(lets say transverse) and if we perform a snare on one polyp and cold forceps on the separate polyp, we can only bill the one with the highest RVU’s. I was always under the impression that as long as there are two separate polyps you can bill for both even if they are in the same area. Has anyone else come across this issue? I am now wondering if this is something that is across the board with all the insurance companies. I have been researching but can not find anything.
