Is this a Detailed or EPF EXAM?

Is this exam Detailed if following 1995 guidelines? I think it qualifies for a EPF on both the 1995 and 1997 guidelines.

In wheelchair
Eyes: Pupils are equal, round, and reactive to light. Conjunctivae and EOM are normal.
Cardiovascular: Normal rate, regular rhythm and normal heart sounds. Exam reveals no gallop.
Pulmonary/Chest: Effort normal and breath sounds normal.
Musculoskeletal: She exhibits edema. She exhibits no tenderness.
Neurological: She is alert and oriented to person, place, and time.
Skin: Skin is warm and dry. Lesion noted. She is not diaphoretic.

Detailed History and Detailed Exam support level 4?

We have a new provider. She always documents a detailed History and Detailed Exam.
Her view is since she is new she is gathering detailed info on all her patients to get to know them. She usually gives an exam with at least 6 elements since these patients are new to her.
Her Medical Decision Making often has 2 or 3 dx without RX.
I’m often finding low level medical decision making.
Would these office visits be scored as level 3 or 4?

Expanded Problem Focused Exam or Detailed Exam? 1995 Guidelines

Hey Everyone:

Using the 1995 Guidelines, to receive credit for a detailed exam, can a provider perform an extended exam for the 1st affected body area and then a limited exam for the next 1-6 symptomatic or related organ system(s)? Or do they need to perform an extended exam for the 1st affected body area and then an extended exam for at least 1 of the next 6 symptomatic or related organ system(s) to receive credit for a detailed exam? Please feel free to include your sources. Thank you!

EPF or Detailed I’m confused

I’m looking at the audit tool sheet here and am confused by this:

1995-1=PF, limited 2-7=EPF, extended
2-7=Detailed, 8+ organ systems=Comprehensive
1997-1-5=PF, 6-11=EPF, 2×6 systems=D
2 from 9 systems=Comp

EPF and Detailed are both 2-7 on the AAPC audit tool in the exam portion. Is this correct? I was always under the impression that 2-4 is EXP and 5-7 is Detailed.

needs detailed contents regarding CSFAC exam

anyone knows the detailed contents regarding CSFAC exam, what subjects are going to be tested (anesthesia, radiology, pathology & lab , ICD10, laws & regulations, ….) and how many questions per subject ? I am going to take the exam in a month. any help would be greatly appreciated. Thank you
