diagnosis coding for rule out auto-immune disease

Hello, I am a coder in a dermatology practice. We ordered bloodwork to find the underlying cause of allopecia. The only diagnosis that we had was the hair loss and insurance is denying the claim for the bloodwork, leaving the pt with a high balance due. IS there a code to link to the order form for ruling out auto-immune disease, or inflammatory causes, or external trauma , or infections?

Use of Z55-Z65 (Social Determinants of Health) as principal diagnosis?

Does anyone know definitively whether or not a patient’s provider may bill Z55-Z65 as a principal ICD-10 code or may they only be used as a secondary code to some other medical/clinical ICD-10 code? Do some payers pay for Z55-Z65 as a principal ICD-10 diagnosis and others do not?

Rash Diagnosis for Biopsy

This is a re-post from last week: Has anyone seen the new LCD # 33445 for Removal of Benign and Malignant Skin Lesions? This is the latest LCD that they added the new biopsy codes to and on that LCD a rash (R21) no longer is allowed to be used as a diagnosis. My physicians use this as a last resort in treating a rash that will not respond to topical medication and if they knew what type of skin condition they would use that diagnosis. Has anyone been able to get this combination paid?

Benign colonic mucosa diagnosis

What should I use for the diagnosis for this?
Screening colonoscopy:
A rectal examination was performed. The Olympus pediatric colonoscope was inserted into the rectum and advanced under direct vision to the cecum, which was identified by the ileocecal valve and appendiceal orifice. The quality of the colonic preparation was good. A careful inspection was made as the colonoscope was withdrawn, including a retroflexed view of the rectum; findings and interventions are described below.

FINDINGS: -hemorrhoids (internal and external), Moderate in size
-polyp(s) – #1, 5 mm in size, Flat, the transverse colon, removed by hot biopsy and sent for pathology

Path report: Transverse colon polypectomy-Colonic mucosa with no significant histopathologic abnormalities.

I would think this polyp would not be coded if it’s only benign colonic mucosa, but it was removed, so if I code only Z12.11, would 45384 be covered? Would the polyp be coded as benign neoplasm D12.3? When I look under polyp/colon/transverse, that’s what it leads to.

Need help with Primary Diagnosis for AWV G0402,G0438,G0439, Ran into a Conundrum.

Hey everyone, I had a rather interesting webinar that was provided by Humana regarding Medicare AWV’s and in the webinar the presenter states that Z0000 and Z0001 should’nt be used, and threw me off, and I was wanting to know everyones thought’s on this, is right or wronge?

Anxiety, Grief and depression diagnosis coding

Hello,

An encounter was submitted with dx:

F33.42 Major depressive disorder, recurrent, in full remission
F43.20 Adjustment disorder, unspecified

But, if patient is noted to have anxiety, grief, and depression, should It have been reported as 3 separate codes:

F41.8-Mixed anxiety and depressive disorder
F43.23 Adjustment disorder with mixed anxiety and depressed mood or F43.21 Adjustment disorder with depressed mood
F33.42 Major depressive disorder, recurrent, in full remission (?)
