Symptoms associated with disease process

Hello,
If patient has anxiety and depression and is on medication for both, and provider is also managing patients sleep disturbance with another med, can I report a code for sleep disturbance? I know the guidelines say not to code for symptoms routinely associated with a condition but since it’s being managed separately can I report the code for sleep disturbance in addition to depression & anxiety?

diagnosis coding for rule out auto-immune disease

Hello, I am a coder in a dermatology practice. We ordered bloodwork to find the underlying cause of allopecia. The only diagnosis that we had was the hair loss and insurance is denying the claim for the bloodwork, leaving the pt with a high balance due. IS there a code to link to the order form for ruling out auto-immune disease, or inflammatory causes, or external trauma , or infections?

Thank you for any suggestions.

Celiac Disease and DEXA

One of the docs I code for ordered a DEXA (77080) for a patient with Celiac Disease (K90.0). Radiology is telling him they won’t use the Celiac code because it won’t be paid. The links between Osteoporosis and digestive diseases exists, but getting those codes paid is another story. Does anyone have any other information on this? I have tried searching the LCD’s/NCD’s, and I can’t find much of anything. My boss isn’t here for me to ask, but she is the one who told me Celiac Disease is payable for a DEXA. Any advice is appreciated, thank you!

Hypertension with Heart Disease guidelines

I need clarification on the Hypertension with Heart Disease combination code I11._. The way I read the Hypertension w Heart Disease ICD10, it relates only to Heart Disease as in codes I50._ or I51.4-I51.9. Is there a combo code for Hypertension with ASHD? I11._ Hypertension w Heart Disease does not appear to include ASHD. I am assuming then, that I would code I10 and I25.10 for example when reporting Hypertension and ASHD. Thanks
