HPI Elements

Need help with clarification of HPI elements. I am not an auditor but the physician has asked me to review some of his charting on E/M codes to make sure the correct level has been billed.

CC: Psoriasis f/u
HPI: 1) scalp, ears 2) + improvement 3) few new spots 4) no problems with current treatment 5) treating w/ T-Gel, T-Sal shampoo, clobetasol solution, and
protopic oint. on face & body

Which elements would you check off for this HPI? I would think Location (scalp, ears) and Modifying Factors (shampoo and prescriptions)

I am unsure if ‘+ improvement’ would be part of modifying factors as the tx is what is causing improvement, or if this would be quality as the quality of the psoriasis has improved. — Lastly, would ‘no problems with current treatment’ be an element of HPI at all, if so, which element?

1995 Exam Elements

Hello,

I’ve been auditing evaluation & management documentation for a long time and I’ve noticed something about the 1995 exam guidelines that I could really use some help on from a fellow auditor or coder or whomever can help!

I get the overall guidelines are based off of body areas and/or organ systems (for 1995), but what I’m having trouble locating is what information counts within each organ system or body area? For example, my providers put any sort of information if it happens to pertain to their head (body area) or gastrointestinal system (organ system). I’ve directed them that it needs to be quality information and not just anything.

As I have understood it, the 1997 exam bullets are the basis for what each body area or organ system needs to contain. For example: The constitutional organ system bullets are three vital signs and the patient’s general appearance. What I extracted from this is that for a 1995 constitutional exam, it needs three vital signs and/or general appearance to count and not just whatever the provider wants to put.

My MAC here in NM is Novitas, and their online audit tool and their audit score sheet only lists the body areas & organ systems to score a 1995 exam. There aren’t any elements or criteria within each one. Is the listing of a body are and/or organ system all it takes to score a 1995 exam? Is there no criterion for what should be listed within the body area and/or organ system?

Hpi elements!!

How do you total the levels once you have determined the HPI, ROS AND PFSH? I am currently taking the E/M training course but for some reason it’s not clicking in my head on how to determine PF, EPF, DETAILED OR COMPREHENSIVE, I feel like there is some step I am overlooking or just overthinking the whole scenario!! Any help would be greatly appreciated!!🙏🏻

We have a provider who lists the same exam elements and findings for every visit in an Internal Medicine Practice. It’s almost like this provider uses a template and doesn’t tailor it for specific complaints or findings particular to each patient.
Is that ok or is that a red flag for an audit?

AWV Required elements

Hello,
I have a provider who saw a Medicare patient for a routine physical and billed 99397, claim got denied by medicare.
we are not able to use AWV codes due to the physical did not contain all the AWV required elements.
Should the provider recall the patient back to perform the AWV elements and re submit the claim using AWV codes.
