Good Morning!

I code for an allergist who has not seen a patient face-to-face in over 3 years. The patient, however, has come in for their allergy shots that have been administered by our nurses. No E/M visit during those times. The patient has an appointment to see our allergist this week. Would this be considered a new patient visit or an established visit? Thank you all in advance!

I’m trying to understand the initial hops care vs subsequent hosp…if the initial care is only for the first visit by the admitting doc why is it that it says “per day” for 99221. I am not understanding something about this and can’t seem to find the answer in searching this forum. If this is not the area to post can someone guide me to the correct area? Thanks.

Established patient codes (99211-15) vs Subseq hosp codes (99231-33) for outpt stay

Question pertaining billing established patient codes versus subsequent hospital codes for an outpatient stay, POS 22:

Why we would use E/M 99212-99215 with POS 22 instead of the subsequent observation care codes 99224-99226? Is there an advantage of using one set of codes over another, or specific rules for why we would use one set over the other?

New patient or established

If a patient is seen in Family Practice and then changes to one of the FP/IM/Ped providers is this considered a new patient visit or an established patient visit? All providers are under the same tax id. In some cases the providers even work out of the same office. In the CPT guidelines it states specialty or subspecialty. Does Internal Medicine make this a different specialty? What if it is a child transferring from one Pediatric provider to another that is Ped/FM/IM?
