Eyes: Pupils are equal, round, and reactive to light. Conjunctivae and EOM are normal.
Cardiovascular: Normal rate, regular rhythm and normal heart sounds. Exam reveals no gallop.
Pulmonary/Chest: Effort normal and breath sounds normal.
Musculoskeletal: She exhibits edema. She exhibits no tenderness.
Neurological: She is alert and oriented to person, place, and time.
Skin: Skin is warm and dry. Lesion noted. She is not diaphoretic.
Is this a Detailed or EPF EXAM?
Is this exam Detailed if following 1995 guidelines? I think it qualifies for a EPF on both the 1995 and 1997 guidelines.

Writing note in books for exam
I am sitting for my CPMA exam in a couple months and I am reading some conflicting info on what is allowed to be written in my code books. Per the FAQs: Handwritten notes are acceptable in the coding books only if they pertain to daily coding activities. Questions from the Study Guides, Practice Exams or the Exam itself are prohibited. Tabs may be inserted, taped, pasted, glued, or stapled in the manuals so long as the obvious intent of the tab is to earmark a page with words or numbers, not supplement information in the book.
No materials (other than tab dividers) may be inserted, taped, pasted, glued, or stapled in the manuals.
So – I want to write specifics on the laws, like the Stark Law and Anti-Kickback Law, such as their differences, fines, ect. Also writing stuff specific to auditing (which I am sure if fine as that is specifically related to daily activities of an auditor). So what do my fellows think about writing notes on the laws/penalties/fines/compliance plans and all that jazz. There’s so much info and I want to be prepared!

Coding Signs and Symptoms not documented in the Assessment portion of Exam
I know you probably see this question a lot. My scenario is a patient is possibly schizophrenia. The rheumatologist provider does not establish medical necessity to support patient is schizophrenia. Provider mentions that patient has memory loss in both the HPI and exam. Is is acceptable for me to report memory loss in conjunction to schizophrenia? Can you please put valid sources?
CPC Exam – Evaluation & Management
In other words, for the key components, do the test scenarios include the specific level, i.e., comprehensive history or moderate complexity, or does the student need to calculate this?
