My providers see patients in skilled nursing facilities and we do not have an actual office location to see patients. We currently bill all payers for professional services for Skilled and ICF (long term care) patients. We are looking to possibly add the Hospice and Memory Care patients at our facilities and want to know if we can still be those services rendered to say Medicare? If there anything different then our regular professional billing with CPT’s 9930-99318? Any insight would be great.

ESRD N18.6 for Hospice
Is there any guideline stating that N18.6 ESRD or any CKD codes can not be used alone? I have researched and with my understanding this category N18.- may NOT be used as primary but must follow the etiology code. But could be used alone if not primary as long as the patient doesn’t have hypertension or Diabetes.

I have a patient who was seen in the home and then transferred to inpatient care for hospice respite. The NP then saw the patient at the hospital-I have been told this patient is to be coded as a home patient, is that correct? Do I code as inpatient?

Injury Code As Primary Hospice Diagnosis
Admitting a new patient to Hospice under the primary diagnosis of Traumatic Brain Injury. Looking for input on the seventh character. As he was already seen at the hospital for this (and is not receiving further treatment due to Hospice admission) I’m hesitant to use "A" as the seventh character. But I’m not necessarily comfortable with using "D" either because of the "routine healing" aspect as he theoretically is not healing, hence the Hospice admission. Any thoughts?
