IPA mammo reimbursement
I am receiving denials for an IPA, specifically La Salle medical associates for mammo codes 77065-77067. They are stating that Medi-cal does not have a fee schedule for these codes, which is correct. Are the G codes payable for 2018? I don’t see them in the Medi-ca; fee schedule at all either. I am not too familiar with Medi-cal so any advice is greatly appreciated!

A patient’s order states Z12.39- screening for breast cancer
This is no the correct screening code for a mammo. Do you query the physician and ask for a corrected DX; or do you code off the description( screening for breast cancer)? I feel like this would be a discrepancy between the code listed and the wording. Could someone please advise.

Medicare is denying the 3D mammo codes
I am needing help with a medicare denial. Medicare had been paying the 77061, 77062, and 77063, but for some reason they are now denying them. I know that the CAD part of a mammo is supposed to get deleted, but that isn’t supposed to happen until January. Can anyone give me some info on why they would deny the 3D? Thank you in advance!
