CPT for Excision of exposed urethral sling mesh

Looking for help for a CPT for Excision of exposed urethral sling mesh via laser destruction.

While the provider was performing cystolitholapaxy for a bladder stone he came across the mesh which "slung across from the 5 to the 7 o’clock position right at the bladder neck, probably about 1 cm to 1.5 cm bridge right there at the bladder neck." Provider tried using endoscopic scissors to cut the mesh but they were not strong enough and therefore used a 550 micron laser fiber to break off the mesh and then finished the cystolitholapaxy procedure.

I am not sure if CPT 57287 is appropriate as it refers to a vaginal approach and this was also done via laser.

Laparoscopic extraperitoneal repair of ventral hernias with mesh

We have a surgeon that is suddenly doing Lap hernia repairs with retro-rectus posterior component separation or Lap extraperitoneal repair of ventral hernias with mesh. Does anyone know how to code these? Do we bill 49652 & an unlisted 49659? Would this be compared to 15734? Would this be considered part of the repair with mesh? Should we bill 49652 with modifier 22. This has us stumped. I have researched for answers but have not been able to come up with anything for how to code these. Anyone familiar with these? Any help would be appreciated.

Subgaleal Siliastic Mesh Placement

Hi folks,

Does anyone know what code I should use to report the placement of subgaleal siliastic sheet "spacer graft"? Does it bundles into the flap?

Provider documentation summary:

PROCEDURES:
Delayed paramedian forehead flap measuring 3.2 cm x 7 cm.
Split-thickness skin graft

** flap was elevated in the subgaleal plane
** skin graft was trimmed to size and secured to the distal flap
** silastic sheet was placed below the graft on the forehead, is a spacer graft.
** silastic sheet was then trimmed to size, placed in the forehead wound bed and the flap was laid back down and secured

I am thinking 15731 for the paramedian flap, 15120 for the skin graft but not sure about the siliastic placement

Placement of Inguinal Mesh without hernia repair

Good morning,

I am in a quandary…any insight will be appreciated.
The provider clearly documents no hernia, no defect, no areas of weakness…but then places mesh anyway?
How can I capture the mesh insertion without hernia repair…am leaning toward 154xx from integumentary system codes but not sure???

POST-OP DX: Right Cord Lipoma
PX: Open Right Inguinal Hernia Repair with Mesh

*inspected the floor of the inguinal canal and identified no defects or areas of weakness
*no hernia sac was identified and the internal ring was well intact
*prior mesh repair of the laparoscopic hernia repair – intact
*identified a cord lipoma distally, and resected that from the spermatic cord.
** then placed a piece of ProGrip mesh and secured to the pubic tubercle

Post Op Note:
informed him that his prior repair was intact and that his bulge was likely from a cord lipoma

57282 with mesh ?

hi, I am wondering… when 57282 is performed and use of mesh, is the Mesh included in 57282? or is there a code to bill for the mesh? I see 57267 but the note in cpt states that it is only billable with 45560, 57240-57265, 57285.

I also posted this in obgyn, a couple of days ago and haven’t gotten any responses yet…

any help would be appreciated.

Coding Help! Laparoscopic Paraesophageal Hernia Repair with Mesh, without Nissen

My surgeon, performed a Laparoscopic Paraesophageal Hernia Repair with Mesh, without Nissen Fundoplication. I am confused as to whether I should use the CPT Code 43282, Laparoscopy, surgical, repair of paraesophageal hernia, includes fundoplasty, when performed; with implantation of mesh. Or to use CPT 43289, Unlisted Laparoscopy procedure, esophagus. I have seen places where it says to use the 43282 even though the fundoplasty was not performed and then I’ve seen where it says to use the unlisted code with the description of Laparoscopic Paraesophageal Hernia Repair with Mesh, without fundoplication. Umbilical Hernia Repair with removal of old mesh and insertion new mesh

I am trying to code an umbilical hernia repair that was done for removal of prior mesh and implantation of new mesh with the hernia repair. The patient had a prior umbilical hernia repair several years ago and now needs the mesh removed due to protrusion from the umbilical skin. Would I code this as an incisional hernia repair with implantation of mesh? I have seen many different opinions of this and need some guidance. I am leaning towards the codes 49560 with 49568.

Removal of mesh and repair of recurrent hernia

Does anyone know of documentation that states that the removal of hernia mesh is included in the recurrent hernia repair codes or that 20680 is not to be used for hernia mesh? One of the surgeons I code for found this article on the ACS and wants to bill CPT 20680 for the removal of mesh. See the clinical coding example section of this bulletin.
