 CMS released the 2017 OPPS proposed rule on July 5 without much fanfare. On July 14, the Federal Register version was posted, and upon initial review, it seems rather short at 186 pages.

Can you bill a repair when you are removing hardware?

I have a patient, that doctor removed painful hardware in the ankle.
two separate incisions so I coded 20680 rt, 20680, 59 ,rt
The note states – prominent hardward was wearing through the deltoid, so when he removed the screw and washer – he repaired the deltoid.
Can I also bill 27698??

Question about removing the A from CPC

I have worked for 8 years for my present employer doing medical billing and coding although I do not have a "coding" position, I work in A/R. Could my employer list my job duties currently correcting claims and looking up codes all day long to clear the apprentice status? Could a prior employer that I did medical billing for be able to give me a letter of my coding experience with them for 6 years? I am willing to do part-time remote coding to help work off the A, but there are no current coding positions open at my job.

Removing High Risk Status

I have been told 2 different things about what it takes for a patient who has been high risk to be removed from that high risk category. I’ve been told that the patient must have no less than 3 consecutive negative pap results, and I’ve also been told that the patient must have 5 years of negative pap results. I can’t find any documentation as to what the true guideline is for this either online or in my coding books. Can anyone point me in the right direction? This patient has a BCBS commercial plan. Thanks!
