Bladder Tumor Resection and Ureteral Stent placement

Can someone please offer guidance?
My provider performed transurethral resection of bladder tumor that invaded the ureteric orifice. He inserted a stent to "facilitate drainage".
NCCI Edits bundle the two procedures together, is it appropriate to unbundle them in this scenario?

"The patient had a large, approximately 3-4 cm papillary bladder tumor on his right lateral wall, obscuring the identification of his right ureteral orifice. This was resected sequentially down to muscle. The right ureteral orifice was identified and it was not the source of the tumor, but did appear to have some involvement of papillary tumor at the orifice. The right ureteral orifice was resected and sent as a separate pathologic specimen. Given the resection, we placed a ureteral stent on this side to facilitate drainage and also assist future resections."

Biopsy of cardiac tumor

What is the correct code to use for taking biopsies of a cardiac tumor when the planned procedure was a repair of right ventricular outflow tract obstruction. Once the chest was opened, an external cardiac mass was discovered and biopsied. I can’t use 93505 because this was an open procedure and we didn’t biopsy the cardiac muscle. Is 33130 appropriate since portions of the tumor were removed or should I use unlisted code 39499?

facial paralysis due to brain tumor – please help!

I am in need of some help with part of a facial surgery one of my physicians performed!
The brief decriptions is "Left preauricular SMAS flap with fixation buccal fat pad and additional SMAS tightening for the correction of facial laxity"
Does any one have a suggestion of what CPT code may be appropriate? I would greatly appreciate any input at all!

PI vs PS Modifier for new tumor

Patient had PET scan under commercial insurance >3years ago for Lung CA, treated and cleared. Now presents with new solid lung tumor under Medicare. Do I use PI or PS modifier for this PET scan? Our coding is leaning towards PS, but as this is a new finding I am inclined to go with PI. Thanks.
