IV hydration with IVP coding

We are starting IV Hydration service at our practice…….If the patient comes in for IV hydration without an IVP the coding seems straight forward. I am confused about if there is an IVP midway. Example:
Normal Saline
start time 0810
stop time 1000

Zofran – IVP
start time 0900
stop time 0901

I am confused about a couple of things.

1. Does the IVP become primary due to the heirarchy?
2. Do I subtract the IVP time from the total time of the hydration?
3. How would this scenario be coded?
4. I am seeing where an IVP should be counted as 15 minutes. Is that only if the actual time isn’t documented? In this case it was a minute.

I have pulled my MAC guidelines and some other info but I am still confused…..
