IV hydration with IVP coding

We are starting IV Hydration service at our practice…….If the patient comes in for IV hydration without an IVP the coding seems straight forward. I am confused about if there is an IVP midway. Example:
Normal Saline
start time 0810
stop time 1000

Zofran – IVP
start time 0900
stop time 0901

I am confused about a couple of things.

1. Does the IVP become primary due to the heirarchy?
2. Do I subtract the IVP time from the total time of the hydration?
3. How would this scenario be coded?
4. I am seeing where an IVP should be counted as 15 minutes. Is that only if the actual time isn’t documented? In this case it was a minute.

I have pulled my MAC guidelines and some other info but I am still confused…..

billing for Hydration code 96361, when not the primary infusion

An IV Push and 2 hrs of Hydration was given at this visit.
Since we need to bill only 1 initial, I chose 96374 for the Iv push and 96361*2 for the 2 hrs of hydration.
Is that correct.?

If hydration was the primary code, then it would be 96360 and 96361.

IV Hydration Coding

Patient presents with Abdominal pain provider orders IV Hydration
96360
J7040

He does not dc the line until the patient leaves about 2 hours later. He orders labs, and CT. The line is KVO according to the note

I explained that according to billing and coding guidelines you can’t bill 96360 with a time of over 2 hours and KVO is not correct.

I explained you can bill 96360 up to 1 hour and than bill 96361 or additional hour.

I also explained that if 96360 is boulos and pt is severely dehydrated you can add another bag of J7040 and bill that out for a full hour.

Can you please assist me. We bill with a place of service 20 Urgent Care

CT with IV Contrast along with hydration

I need some help please. So a CT of the chest with IV contrast was done. Then after that, hydration was given for 45 minutes. My understanding is that you would not bill for the administration of the IV contrast as it is included with the CT. So my dilemma is coding for the administration of the hydration. Would I use the initial code 96360 or the secondary code 96361? I have tried it both ways and have gotten denials from BCBS for both ways. I have tried 96360-59 and I have tried 96361-59 and I have also tried 96361 without a modifier. Nothing seems to work. Can anyone tell me what I am doing wrong?

confused (hydration)

Hydration seems to be a mystery, with all my research so many conflicting opinions as to how hydration is to be coded correctly. I have some opinion stating hydration cannot be billed and some others stating all can be billed in this scenario. Is anyone able to offer their knowledge please?

magnesium sulfate IVPB 2 g/50 mL Premix 0940 1030
sodium chloride 0.9% bolus 1,000 mL 0940 1110

Note states only treatment: Hydration. What is the correct coding of hydration.

IV hydration billing for OB/Gyn office

My physicians are interested in starting to bill for IV hydration instead of sending our patients to the ER, especially for OB patients. I have never coded or billed IV hydration…….. can someone please help me??? What CPT codes and diagnosis would I use? What are the guidelines and where can I find them?:confused::confused:

Concurrent Hydration

Hello! Hydration help would be much appreciated!!!

I know concurrent hydration cannot be charged when hydration is run with a drug, but I can’t find anything about when two hydrations are run together through the same line. Below is what I’m referring to from my L&D department:

Lactated Ringers bolus 1,000mL: 10:00 – 17:00 Total of 7 hours
Lactated Ringers Infusion: 12:00 – 17:00 Total of 5 hours

My question is how many hours of hydration can be charged? 7 hours, 12 hours or 2 hours?

Hydration or Therapeutic Administration Question?

Hello. I have a question in regards to hydration and infusion coding.

Here’s the scenario:

Patient came into the ED with abdominal pain and had developed nausea and vomiting. The MAR states "Dextrose 5%-0.45% NaCL 1000 mL + 20 mEq KCL Intravenous at 125 ml/hr." Would this be coded as hydration or as a therapeutic infusion? Usually when I see "vomiting" in the medical record, I automatically think hydration, but with potassium and dextrose — would this be therapeutic? So confused! Thanks for your help!

Iv hydration

Hi I work in a hospital and I am working on auditing IV hydration therapy coding. I haven’t worked on this area in a long time and trying to understand it, but seems like I am not getting it (or overthinking it). I have an example of one which this example is really confusing me.

1. Dilaudid: IV 18:41-21:11
2. Zofran: IV 17:50-21:11
3. Sodium Chloride IV: 17:50-21:11 (201 min)

1. 96375
2. 96374
3. 96361X3

I have read a lot of notes (from AHIMA) and other web pages but it seems like I am unable to get this through my brain. Any help is appreciated. Thank you.: :confused:
