billing for Hydration code 96361, when not the primary infusion

An IV Push and 2 hrs of Hydration was given at this visit.
Since we need to bill only 1 initial, I chose 96374 for the Iv push and 96361*2 for the 2 hrs of hydration.
Is that correct.?

If hydration was the primary code, then it would be 96360 and 96361.

Reporting multiple 96361

We reported a 96360 and 2 lines of 96361 add on codes. One of the 96361 denied by Medicare as a duplicate. I do not see anything in the book to indicate that a modifier would need to be used. Can anyone tell me if we are to report multiple units and not multiple lines? I have never reported multiple units on any service except for medications. Thank you in advance.

Hydration Infusion Codes 96360 vs 96361

Our office did 35 mins of hydration prior to the start of IV infusion, would we bill 96360 since it was 35 mins or because we are using 96413 would that be considered the primary procedure would we use 96361?
