Critical care 99291 coding – time

Hi All,

Not sure if I’m in the right area, I’ve asked for an ICU/Critical Care group but never got an answer from the powers at AAPC

Question – Are your providers adding their "time to document chart" to their critical care time. This has just come up recently and we are getting some that do and some that don’t. The description for 99291 doesn’t specify either way… ??

First hour of critical care of a 65-year-old man with septic shock following relief of ureteral obstruction caused by a stone.

First hour of critical care a 15-year-old with acute respiratory failure from asthma

First hour of critical of a 45-year-old who sustained a liver laceration, cerebral hematoma, flailed chest, and pulmonary contusion after being struck by an automobile.

First hour of critical care of a 65-year-old women who, following a hysterectomy, suffered a cardiac arrest associated with a pulmonary embolus.

First hour of critical care of a 6-month-old with hypovolemic shock secondary to diarrhea and dehydration

First hour critical of a 3-year-old with respiratory failure secondary to pneumocystis carinii pneumonia

G0390 (Trauma Team Response) denied when billed with CPT 99291 & ER Services

We have several claim that are being denied payment for G0390, Trauma Team Response with the correct Rev Code 689 billed on the same day as CPT 99291, Critical Care. The EOB always says that the CPT/HCPCS is not valid/ correct or that services were not performed. The payer is WV Health Plan Medicaid. I have reviewed the CMS guidelines and it looks like it is being billed/coded correctly. Is anyone else having an issue getting G0390 paid?

G0390 with 99291 Trauma Billing- Help

Does anyone use the G0390 on professional claims? It is in the HCPCs book and per what I can find online it needs to be billed with Level 1 Trauma activation, but only if critical care is performed. Our billing dept is saying the G0390 is only billed on the facility side?

Any help would be greatly appreciated!
