Recent discussion on a denial for age specific diagnosis of T74.4XXS (Shaken syndrome) on a patient that had reached the age of 21. So, the question is at what age does the patient move from an active pediatric code to a ‘personal history of’ code? I can’t seem to find a reference any where on this specifically.

Parents Making Appointments For Their Adult Children
I believe that it’s a HIPAA violation to allow a parent to make a medical appointment for their adult child (over 18). We are a pediatricians office so this happens regularly – and usually a month or two after the child turns 18. I’m getting pushback and would like to gather other feedback. Please advise.
CPT for double dose of Recombivax B pediatric/adolesc strength given to an adult
When we are out of the adult strength Hep B and an adult needs a vaccination, we use two 0.5 ml vials of Recombivax B, pediatric/adolescent strength (5mcg/0.5ml), in one 1.0 ml injection so that the adult patient gets the proper adult dose of 10mcg. Billing for this has always been a challenge. If we bill 90744×2 some insurance companies deny the claim as an age mismatch. If we bill 90746 the claim may deny because the NDC does not match. What is the correct way to code this?
ADD – Adult
Hello – I have an adult with ADD and am trying to find a code. When searching for ADD in the index I see Attention-deficit without hyperactivity (adolescent) (adult) (child) F98.8. However when I look in the tabular list F98.8 states P (pediatric) code. Payers deny this code due to age when used on a claim for an adult. Any suggestions?

Adult dorsal slit code
I would like to know which cpt code should be used for a male adult dorsal slit..physician used a clamp on the scar to perform the dorsal slit. I used cpt code 54161. Is this correct or should I be using cpt code 54001?
