Low Dose Lung CT Certification

I had a client in IL ask me today where were are able to upload the Certification once certified for this screening. They weren’t for sure if it would be in PECOS or not. I checked today and I do not see the option for these as you do Mammo Certification. For those of you who have been certified where did you upload your certificates?

Denial for Flu vacc with MVP NDC dose requirement

Wondering if any one else has had denial for flu vaccine MVP request dose given we submitted the claim with the 11 digit NDC 58160090552 ML0.5 and received a denial stating decimal precision. Value submitted 0 error code M (missing)
Is there a different way to submit when dose is required ?
appreciate help in correcting

G0296 – Counseling visit to discuss need for lung cancer screening using low dose ct scan (ldct) (service is for eligibility determination and shared decision making)
G0297 – Low dose ct scan (ldct) for lung cancer screening

CPT for double dose of Recombivax B pediatric/adolesc strength given to an adult

When we are out of the adult strength Hep B and an adult needs a vaccination, we use two 0.5 ml vials of Recombivax B, pediatric/adolescent strength (5mcg/0.5ml), in one 1.0 ml injection so that the adult patient gets the proper adult dose of 10mcg. Billing for this has always been a challenge. If we bill 90744×2 some insurance companies deny the claim as an age mismatch. If we bill 90746 the claim may deny because the NDC does not match. What is the correct way to code this?

High dose Influenza vaccination

A high dose flu shot was given to a pt who is under 65 years old due to the patient is immunocompromised. Insurance has denied it as not eligible due to the patient’s age. Would this be appealable or they would be denied regardless? Does anyone have came across the same issue? Any feedback would help! Thank you!
