Diabetic Eye Exam in PCP setting (Internal Medicine)

I work in a Primary Care office and my physician is thinking about purchasing the machine/software to perform diabetic eye exams. What would be the correct CPT code since it’s being done in the primary care office setting? We’re needing to check on reimbursement so I want to be sure I have the correct codes.

I would really appreciate any feedback if any of you have experience with this! Thanks in advance!!

Diabetic combination codes

If you have a diabetic patient who is hypertensive, with stage 3 kidney disease, is the assumption still that the kidney disease is related to the hypertension? If so, would I still bill the diabetes with renal manifestations?
Does the provider have to link the chronic kidney disease to the diabetes in order to use the E11.22 code?
Do they actually have to say "due to" or "Because of" type wording before you can combine the codes.
I’m having a hard time understanding this.

Billing Type II diabetic eye exams

I recently received a denial from an insurance company for "invalid second diagnosis code". The patient has Type II diabetes and code E11.9 was used.
The 2017 ICD-10 book shows that you need to use an additional code to identify "control using" …either insulin or an oral drug. I have corrected this code but the question that came up is – what do you use if the patient is diet controlled? I believe it will be denied if I don’t have a second code now and I have been unable to come up with an answer. Thank in advance for any advice.
