Medical Nutrition Therapy

Here is my , scenario:

My practice provides Medical Weight loss for obese patients, we have CRNP’s the provide a E/M and also Preventative counseling 99401-99404.

We also have Certified Dietitians who provide medical nutrition therapy: CPT 97802 for NEW initial assessments and 97803 for re-assessments follow up for new patients. So here is my questions.

1.Do we bill the CRNP portion separate from the RD’s (same TAX ID) on the same DOS
2. There isn’t alot of documentation under MNT services 97802-97803 in the CPT book (No Guidelines) just that a 97802 must be performed only on NEW pt’s to the practice and the 97803 can NOT be performed under a pt that hasn’t had and assessment 97802
3. The DX codes we use for the NP is a obesity code E66._ followed by Z71.3 and a BMI code
4. The only DX we can bill with the RD’s is Z71.3 and a BMI DX code.

Please someone help us we are drowning :(

just reaching out to see if anyone can direct me to an auditing checklist for a Nutrition office that sees Medicare patients. I am trying to help a small practice with their new group sessions where they see Diabetic patients who are covered by Medicare. Any info you can share will be greatly appreciated!

Hello All~
Can someone please direct me to where I can find a policy or information on billability requirements for MNT for a NON-MEDICARE patient who is SELF-REFERRED? I see a lot on Medicare requirements and that we are unable to bill for MNT services WITHOUT a Provider referral and covered diagnosis.

This non-medicare patient of mine is SELF-REFERRED and doesn’t want regular insurance co billed. (No PROVIDER referral!) Are we allowed to bill the patient WITHOUT a provider referral if they want the services? What if they also sign a waiver of payment responsibility? Or even if they are a Medicare patient, knowing they don’t cover MNT for non-DM diagnoses and sign an ABN or waiver? Again, all of this is in regards to the referral issue.
My company is stating that ALL MNT services require a provider referral whether self-pay, signed waiver, or not, but I feel that if anyone wants this service and is willing to pay out-of-pocket for it, it shouldn’t need one. Please help if you can. Thank you so much!!!
