diagnostic lung biopsy with cpt 32124

Can someone please help me with the following:
I bill for a thoracic surgeon and he consistently performs code:
32097 Thoracotomy, with diagnosis biopsy(ies) of lung nodule(s) or mass(es)
and on the same surgery sessions performs:
32150 Thoracotomy with the removal of intrapleural foreign body or fibrin deposit
and 32124 Thoracotomy with open intrapleural pneumonolysis.
My question is when I used to bill these codes together CPT 32097 will get bundled, but when I added modifier 58 staged procedure to CPT 32150 and 32124 it was paid, so how is this a staged procedure? and when will be adding modifier 58 correct with these codes?
I understand that if it was a biopsy followed by resection or an excision, but what does a pneumonolysis have to with the biopsy? I also know that I can not bill a diagnostic biopsy(ies) of the lung regardless of the approach if during the same operative session the surgeon uses these results to determine the extent of the necessary surgical resection.

Low Dose Lung CT Certification

I had a client in IL ask me today where were are able to upload the Certification once certified for this screening. They weren’t for sure if it would be in PECOS or not. I checked today and I do not see the option for these as you do Mammo Certification. For those of you who have been certified where did you upload your certificates?

Do any private health insurance companies reimburse for these procedures?

G0296 – Counseling visit to discuss need for lung cancer screening using low dose ct scan (ldct) (service is for eligibility determination and shared decision making)
G0297 – Low dose ct scan (ldct) for lung cancer screening

metastatic melanoma of the lung

We have a patient referred for metastatic lung cancer for radiation therapy. Bx was performed and came back s suspicious for metastatic melanoma with strongly positive S100 and SOX10 showing weak staining with Melan A. Origially coded as C78.01. From a coding standpoint, what is the difference between code C43.9 and C78.01? Both have refernces to including Secondary malignancy to the lung. Since there is no outward clinical evidence of there being Melanoma to the Eyes or Skin which would be the more appropriate code to use?

Lung Biopsy

Coding for a VATS with lung biopsy. The surgeon took a biopsy from upper, middle, and lower lobes of left lung. The code 32607 is for lung infiltrates, 32608 is for biopsy of lung nodule or mass, and code 32609 is for the pleura. None of these seem appropriate to me. Am I missing something or is there not a code for biopsy of the lobes of the lungs. Please help.

Lung plug device?

Is anyone using a lung plug device and coding additional services or a 22 modifier for the radiologist’s work? They are stating it’s "extra work" but I’m not convinced.

I have the reimbursement guide, but not helpful for professional radiology coding, and I cannot find anything on how to code (other than the C2613 for facility), or if indeed it would be considered extra work. I am thinking the only advice I can give the docs is to document that "extra work" thoroughly, add the 22, and see what happens on claim submission. Any thoughts or real-world experience would be helpful. Thank you!

Seeking help for transdiaphragmatic approach to lung wedge resection

My surgeon performed a wedge resection of the lung using a transdiaphragmatic approach. however he went in through an incision already made by another surgeon that removed the diaphragm.
The only codes i am familiar with are VATS, wedge or thoracotomy wedge resections. Any advice?
