Help Needed: Project and Research Question-Correct an Edit

I am stumpt on this preoject/research question below, any assistance would be so greatly helpful :) Thanks so much, Brittany-

CPT code 77003 is present under revenue code 320. CPT code 64490 is present under revenue code 360. An NCCI edit is present on code 77003 and reads: “Warning: Do not report CPT code 77003 in conjunction with 22586, 27096, 64479-64484, 64490-64495, 64633-64636, 0195T, 0196T or 0309T. According to the medical record, 64490 procedure was performed under fluoroscopy. How would you correct this edit?

Help with MUE Edit 1 – Claim Line Edit

Hello, I’m looking for some help with Medicare MUE MAI 1 policy for claim line edit. I always took this to mean anything units above the MUE needed to be billing on a separate line.
Example, 88341 x12 units, billed as 88341×9 , 88341×3. I am still getting denials for the additional 3 saying they exceed the maximum allowed. I’m using correct X modifiers as well! How is this a line edit?? Any help is appreciated.

Modifier 59 for duplicate service- no mue, no cci edit. I disagree.

Would like everyone’s opinion with this situation. I have a series of CPT codes reported on the same encounter, same patient, ect. There are duplicate service codes reported for one service. The MUE is 2, and there are no NCCI edits when coded with the other services on the same claim.
A colleague of mine is adamant that we should report that code, on two different lines, with a modifier XS on the second line.
My argument is, that CPT code, has a MUE of 2, is not subject to the multiple payment policy indicator of 2, nor the payment policy indicator of 3, and has no bundled edits, therefore, we should be reporting that code, one one line, with a total unit of 2 in lieu of reporting the XS modifier, and splitting out the code.

Any takers on this? I am fully aware of Medicare’s guidelines with regards to the 59 modifier, but I do not see anything that can back up my analysis of this stating we should not be reporting this way, because it is improper coding, and can trigger an audit.

[Announcement] NCCI Has Removed the Current Edit Prohibiting 77295 and 77300 from Being Reported Together

CMS and the NCCI has removed the current edit prohibiting the reporting of CPT codes 77295 and 77300 together, effective July 1, 2016. The change will be finalized in the July version of the NCCI Manual and will be retroactive to January 1, 2016. Reporting requirements may vary by payer:

Claims reported to Medicare contractors: 

Claims for CPT codes 77295 and 77300 should not be reported together until the edit is removed on July 1, 2016. Practice billing systems can begin capturing charges for 77300; however, the charges should not be released to Medicare until July 1, 2016. All prospective and retrospective pending charges for 77300 may be released on or after July 1, 2016.

    Claims reported to private payers:

    Depending on state guidelines and individual payer policies, some private payers may not permit claims to be submitted after a certain period of time has passed. Therefore, practices could consider submitting charges for 77300 to commercial payers prior to July 1, 2016. Refer to your payer’s individual reporting policies for guidance on when to submit claims for 77295 and 77300.
