fracture coding for elderly patient

hello,

Would someone be able to clarify the fracture coding confusion for me please?

65 years old female came to ED complaining for R arm injury after she tripped and fell. No past medical history. ED provider diagnosed patient with R distal radius fracture.
My question: how this should be coded: Right Distal Radius Fracture or, osteoporosis with right distal radius fracture because of this patient’s age?
Per guidelines- code fracture with osteoporosis when osteoporosis is documented. However, I found an article where the fracture can be coded with osteoporosis because of the patient’s age.

Any input will be appreciated.

Coding ICD-10 009.52_ Supervision of elderly primigravida or elderly multigravida

Can the Supervision of elderly pregnancy code (009.5__) be assigned based on patient’s age of 35 and older even if the documentation does not state Advance Maternal Age? I was told by a coder that the provider must document Advance Maternal Age in the progress note to assign this code?

Coding ICD-10 009.5– Supervision of elderly pregnancy

Can you assign the Supervision of elderly pregnancy code (009.5__) if the provider does not state in the medical record Advanced Maternal Age? ACOG states that is patient is 35 years and older at expected date of delivery, this DX can be assigned. Some coders argue that the medical must state "Advanced Maternal Age" to assign.

Elderly Primigravida in Emergency Department

I have some confusion regarding Pregnancy coding in the Emergency Department.

If it is the provider’s responsibility to document that a pregnancy is incidental to the Emergency Room encounter, I assume that all encounters that are not documented as such are to be coded from Chapter 15 of the ICD 10.

If the patient is 38 years old, would it be appropriate to use Supervision of High Risk Pregnancy – Elderly Primigravida or Multigravida on the ED encounter, or is "Supervision" for use by OB/GYN?
