Does anyone know if direct supervision is required for a PA or NP to administer chemotherapy infusions? I have been told it differs by state but cannot find any documentation to support this. If anyone has documentation I would appreciate it if you could send it my way.

My physician is considering billing for 91122 done/performed by his MA, so can we do that without his direct supervision, as code description states: performed by physician?

I am having difficulty finding answers to my question and I am hoping that I may be able to get help here. I am trying to find out if a LCSW can bill for their services or if the need to have supervision by a LICSW in order to meet the LICSW requirements. To clarify, our providers are asking if a LISCW can bill for patients the LCSW sees uder her supervision. I am assuming the answer is no but am looking for guidance.

Coding ICD-10 009.52_ Supervision of elderly primigravida or elderly multigravida
Can the Supervision of elderly pregnancy code (009.5__) be assigned based on patient’s age of 35 and older even if the documentation does not state Advance Maternal Age? I was told by a coder that the provider must document Advance Maternal Age in the progress note to assign this code?
Physician supervision
I’m being questioned again about physician supervision. Can anyone tell me if you can bill for fluoro’s done in the OR during a fracture fixation or an ERCP done by the gastrologist. The radiologist isn’t in the OR for either of these. And if anybody can supply me the Guideline info it would be greatly appreciated.
resident supervision with GC and GE modifier
I code physician services for a hospital that has an internal medicine and surgical residency program, the company I work for was instructed by hospital administration not to use the GC modifier with the logic that the hospital has an outpatient clinic GME program therefore all In patient services are exempt from reporting the GC modifier. My understanding is that any service done by a resident under supervision needs the GC modifier and GME programs only cover low level outpatient EM’s when done by a resident without direct supervision but still require modifier GE. Guidelines from CMS have been presented to the hospital administrator and she is admit that reporting supervised resident services with GC modifier is incorrect, that no modifier is needed can anyone advise?
Supervision of pregnancy following previous miscarriage
How would you code the supervision of a 25 week pregnancy, for patient who has a history of a previous miscarriage? Would you code O09.292, O09.899? Or would you not go high risk at all for the pregnancy? Here is some information from the ultrasound report. I am being instructed that O09.292-supervision of pregnancy with poor reproductive or obstetric history is the correct code. But that seems more severe than what this patient’s history indicates so I wanted to get a second opinion from the group here as I dont code obgyn regularly.

Can you assign the Supervision of elderly pregnancy code (009.5__) if the provider does not state in the medical record Advanced Maternal Age? ACOG states that is patient is 35 years and older at expected date of delivery, this DX can be assigned. Some coders argue that the medical must state "Advanced Maternal Age" to assign.
Supervision of Hemodialysis-Inpatient Facility
What type of supervision does hemodialysis require in an inpatient facility setting? The physician will consult with the patient then order H.D. but there is no documentation as to the supervision of the service. I have looked at Medicare Conditions of Participation and IL state law. Still can’t seem to find anything solid??

Attain Effective Weight Loss under Medical Supervision
