Hypertensive Urgency

My coder came to me saying that the doctor is not using hypertensive urgency for a patient who walked in with 190 mm Hg blood pressure.

Our doctor just dictated it as hypertension, severe (I10).

Is there a standard range limit wherein it can just be hypertension (severe) vs hypertensive urgency?

Coder means well, goes by the book but sometimes gets a bit too enthusiastic about being the coding police.

Dx coding guideline resources for DM Retinopathy and Hypertensive Retinopathy

I need resources to train my team of coders on DM retinopathy as well as Hypertensive Retinopathy. If anyone could provide links to documents that would be great. Also, if the PT has DM and the provider states that the PT has age rellated NS cataracts, do you still code E11.36 as well as H25.XX? Thanks in advance.

Hypertensive Heart Disease

My coder attended a seminar and was told that if patient has had Hypertensive Heart Disease (w or w/o heart failure) that it never goes away and should be in all the subsequent visits’ diagnosis and assessment list.

Our EMR puts past dx in the history and lets the provider activate it as needed. Our provider would deem its Hypertension and skip the HHD dx. Our coder comes back insisting that the provider click HHD. Our provider is in the opinion that she could code it from the past med hx.

Who is right?
