How to code diagnosis diabetes mellitus with A1C

I have had 2 different doctors in the past week list their patient’s elevated A1C with the diabetes diagnosis. Listed in the Assessment, the doctor wrote as the patient’s diagnosis "Diabetes Mellitus, Type II with A1C 8.7". For the 2nd doctor, he wrote the unforgivable…"Uncontrolled diabetes, A1C, 7.6". I understand that we cannot code from lab values but both doctors are clearly trying to express the patients have DM with hyperglycemia. In both H&Ps, the doctors discuss the patients needing to watch their diets, loose weight, etc, etc. So what should I code these as? E11.65 or E11.9?

Rationale/opion given is that the physician has linked the DM and CKD utilizing the word "with" and coding guideline for "with" should be interpreted as "associated or due to"

Hi can anyone help me on how to code this

Type 2 diabetes mellitus with hyperosmolar nonketotic hyperglycemia

am confused whether to use E11.69 Type 2 diabetes mellitus with other specified complication or E11.65 Type 2 diabetes mellitus with hyperglycemia; I cannot code this to E11.00 Type 2 diabetes mellitus with hyperosmolarity without nonketotic hyperglycemic-hyperosmolar coma (NKHHC) because coma is not documented.

Diabetes mellitus post liver transplant.

Hi,

The patient has cirrhosis for which the patient underwent liver transplant in 1999. The patient later in the year of 2000 developed diabetes mellitus. The physician is stating DM(post liver transplant). What diagnosis code for diabetes mellitus post liver transplant be appropriate.
