Post infarction angina. paging mitchellde

Patient was admitted to the hospital with shortness of breath, tests showed elevated troponin. He has a history of old infarct involving old inferoapical area.
Doctor had the dx of NSTEMI, Type 2. Cath a few days later showed mild diffuse disease of LAD, RCA, and circumflex artery.

Patient came back to our clinic 27 days after hospitalization, initial dx of NSTEMI.
Doctor had dx CAD.

Coder came back saying that she will be coding it still as NSTEMI and complaining why the doctor never mentioned it is still NSTEMI.
She also said that if patient has angina, she would need to code it as post infarction angina.
Doctor had explained that medically post infarction angina is a rare occurrence in 10-15% of cases.

I felt like CAD w stable angina I25.118 as documented is more appropriate.

The doctor has felt that CAD would include the new mild arterial diseases and the old MI.

Is CAD appropriate to code?

icd-10-cm code for vaginal cuff dehiscence post hysterectomy

What’s the best diagnosis for a vaginal cuff rupture/dehiscence? This is about 3 months after hysterectomy, op report says "vaginal cuff open approximately 1 inch with bowel present at the cuff edge.". I keep finding N99.3-vaginal vault prolapse after hysterectomy– but that doesn’t sound right to me.

Wound Vac/Negative Pressure Therapy post op period still in house

Hello Everyone,

I was wondering if anyone could offer any guidance please. I have a Dr. that did 2 debridements on a pt that is in house.
On their rounding days they are applying a would vac/Negative pressure therapy. I was under the impression that this
would get bundled into the 90 day global. I know that the Hospital (PT Department) can charge for it but the Dr. is wanting
to charge for it. Any feed back would be greatly appreciated!

Billing Post op visits to Workmans comp carriers

I work for a group practice where two of the MDs use a separate billing company. That company bills post op visits (99024) to Workman’s compensation carriers with a charge of $ 250.00 and are getting paid!!! Has anyone heard of this? I want to get on top of this before the other MDs catch wind and want to do the same. I do not feel its proper or even ethical, at that, if they are following the rules of CPT. The treatment after the surgery is included in the surgery code for a certain time frame. Any thoughts?

AMA Pre, Intra, Post Service Times

Hello,

Does anyone happen to have the a link to the AMA Pre, Intra, Post Service times for procedures and EM services?

I know that this is put out by the AMA RUC Committee and is used by CMS for RBRVS, but cannot find the actual document that contains these times.
I have tried to email the AMA but have had no luck.

If anyone happens to have this information it would be very much appreciated!
