Obtaining Correct POS From Facility SNF/Assisted

My Dr does rounds at a local what I thought was a SNF, I billed 99305 with POS 31 the address of the facility and the referring physician but still recieved denials from Medicare. Do I continue to try and get ahold of someone to ask at the facility or do I call and ask Medicare?

Obtaining Diagnosis Over the Phone

1) Patient receives a bill from the pathology laboratory and contacts his provider. The provider then phones the lab to provide additional diagnosis codes to re-file a claim. Is the lab obligated to obtain a written confirmation of the diagnosis obtained via the phone call?
2) Provider orders laboratory work on a patient but does not provide a diagnosis on the lab order. Can the lab staff phone the provider for a verbal diagnosis if the claim has not yet been processed for adjudication? Is the lab required to obtain a written confirmation of the diagnosis obtained via the phone call or can the lab merely document the additional info/diagnosis onto the patients lab request?

Do we need to code 51701 for obtaining urine sample just for urinalysis??

Hello Everyone,

Can anyone help me with this please…

Patient is 10 months old and straight catheter was used to obtain urine sample for urinalysis..so my doubt is do we need to code for 51701 in this scenario??

As per encoder straight catheter is for residual urine.
