We had a Conversation going on in our office regarding usage of 25 modifier along with E&M 99213 (Outpatient visit) Were patient came with the complaint of Painless bleeding during bowel movements after examining the patient physician found that patient is having Bleeding internal hemorrhoid were he decides to do a Hemorrhoidectomy by rubber band ligation(s) on the same office visit and performed the procedure

Dx codes: K64.8

99213-25 ? -K64.8
46221-K64.8

We are being advised that we cannot use 25 modifier for the above scenario

Anyone with feedback on this would be greatly appreciated..

How to Be Sure Your Radiology Documentation Supports Proper Coding for Moderate Sedation

The Medicare Physician Fee Schedule (MPFS) for 2017 contained some revisions to the coding and reimbursement for moderate sedation that will potentially impact radiology practices. Previously, moderate sedation was not separately reimbursed for many of the procedures listed in Appendix G of the CPT®[i] codebook.  Sedation was bundled with the basic procedure, but now it will be reimbursed in addition to the basic procedure.  There are important guidelines and documentation that must be met in order to use these codes, and radiologists interested in maximizing their reimbursements should review their reporting to be sure it supports proper coding under the new rules.

Proper billing for separate procedures

I have a provider, who is paid by RVU’s, and he is wanting to bill out multiple procedures on separate HCFA’s. Example, he does two different procedures, and instead of billing both out with 59 modifier on second procedure, he wants to bill out each procedure on separate HCFA. The payment modifiers change how his RVU’s are counted. If the procedures are billed out separate, in his mind, you don’t have to put the 59, thus giving him 100% value for both procedures. I have looked through CMS guidelines and I don’t see anything stating if more than one procedure is done in the same day, that they all have to be billed out on same HCFA. If anyone knows where I can find some guidelines in regards to this, I would greatly appreciate it. The provider is saying it is incorrect to bill them all out on same form, so I just need something in writing from CMS to show him otherwise. Thanks in advance for any help!
