Can the documentation for the PFSH just say Reviewed, IF there is intake paperwork scanned in the chart that the patient filled out? In the real world, will an auditor not give credit there or would it be ok if there really is paperwork filled out by the patient and scanned in their chart?

Data reviewed from Xray/MRI
If your coding pain mgmt., the Dr documents his final dx radiculopathy of lumbar , however you notice under data reviewed he documents the MRI/XRAY results as per the radiologist DDD of lumbar.
Can I code from the data reviewed / MRI/XRAY interp? and make the code DDD with radiculopathy even though he isn’t saying DDD in his final

For hospital progress note (subsequent care), attending physician documented "consult reviewed". Does it credit towards to MDM complexity of data to be reviewed?
MDM- “Amount and/or Complexity of Data Reviewed”
I am in need of some advice on some records I’m reviewing. This is directly related to the "Amount and/or Complexity of Data Reviewed" of the MDM component.
If the Physician notes she performs a "Urine Toxicology Screening", checking for RX compliance, and mentions she personally "reviewed the dipstick herself which provides a preliminary result for the potential of more or more drugs", how many points does this qualify for?
One total, for the review and/or order of clinical labs?
Two total, for the independent visualization of a specimen?
Three total, for ordering/reviewing and also independent visualization?
I’ve seen many conversations about this, but no hard documentation that says truly which one. I’ve researched in Optum’s Evaluation and Management Coding Advisor and it isn’t answered for my scenario. I’ve Googled and can only find opinions, but nothing from bigger reputable sources. If anyone has something in writing I would greatly appreciate it!

Modifier on x rays ordered by a PA and reviewed by the physician
Medicare is now denying the new chest x ray codes when the patient is seeing a PA. The PA documents that the physician reviews the x ray. What modifier am I required to append to the claim? Anything else I need to know?
What is the average number of provider’s reviewed for E/M compliance
I am being asked for information regarding productivity of our auditors. Does anyone know what the average number of provider’s reviews for E/M and compliance elements per auditor per year. I am having difficulty finding those numbers and thought perhaps someone has some information or reference for me. Thank you
E/M MDM: Data Reviewed
I’m really getting confused regarding giving data points for Labs/Rad procedures ordered and interpreted by physicians.
If a physician interpreted results of a lab test or rad procedure ordered(and charged) from a previous visit(but of same service) and then ordered another lab/rad during current visit, can we give points under "independent visualization" AND "review/order of lab/rad test"? or is this only considered under review of lab/rad?
