Treament options or number of diagnosis

Just for clarification, if a patient with no symptoms presents for a consult for screening colonoscopy, would this be scored as self-limited or minor, or new problem additional work-up planned. There is some disagreement in our office about this. Or if anyone could point me to a source to be able to have concrete rules about screening consults that would be great too. Thank you!

I see that 1 point is awarded for "self-limited or minor (stable, improved or worsening)"
I see that a "new problem to examining MD with no add’l workup" is 3 points
How do we determine whether to count chief complaint or new symptom or acute problem as self-limited or minor,
or new problem with no add’l workup?
New problem to examiner or new problem to patient?

What is the average number of provider’s reviewed for E/M compliance

I am being asked for information regarding productivity of our auditors. Does anyone know what the average number of provider’s reviews for E/M and compliance elements per auditor per year. I am having difficulty finding those numbers and thought perhaps someone has some information or reference for me. Thank you

Her exams are always level 4 with at least 6 body/organ systems for detailed level 4 exam. History sections vary.
Would visits with level 4 detailed exams and 3 diagnoses under Amount/complexity of data be all level 4? with key factor being at least 3 dx listed in assessment? Assuming table of risk is moderate/level 4 (Prescription drug Management) also assuming 1 or less points under Amount and complexity of data (low or minimal)
