Excision & repair coding sequence

Just starting out with my CPCD coding course. I’ve been working on the practice case studies, and I’m finding that a lot of the repairs are being stated as the "first CPT listed". When reading the case studies, the first CPT listed was (what I thought would was) the procedure of the excision itself. What is the sequence of CPT codes listed when coding an excision and repair for example? Are repairs always going to be listed before the procedure itself? Also, where would I find these guidelines stating the info I’m looking for? Thank you!

Home Health Dx Coding Sequence

I have recently started working at a home health company and am doing some coding. My question is the sequence of the dx codes. If I am looking at it from the home health agency perspective, would the primary code be the "reason" for the home health or therapy the patient is receiving? For instance we had a patient that had vertigo and muscle weakness, and also a UTI. She was receiving physical therapy. However, the company had sent this chart out to an outside coding company and they came up with the UTI as the primary diagnosis. I did not see how that would be appropriate. I would think the vertigo and then muscle weakness would be the first two codes as this was the reason for the therapy. What do you think?

Z code sequence question

Hello, looking for help from Medicaid payer – per ICD-10 NCCI Manual, a certain list of Z codes (below) must appear in the primary dx position. Most if not all are encounters – question: if a STI (Sexually Transmitted Infection) screening Z code appears on the same line, does it matter which (encounter or screening) position the screening Z code appears in? And is it correct to bill an encounter and a screening Z code at the same time/same day? Thanks.
