Diagnosis Codes for Annual, Pap smear, hysterectomy, hx cancer

Which diagnosis codes do you use for Preventative GYN visit 993xx and 88175 Pap smear, patient has had a hysterectomy due to endometrial cancer and vag Pap is done. Would Z01.419, Z90.710, Z85.42 be correct?

Would the same diagnosis codes used for this for both commercial insurance or Medicare?

CPT vs HCPCS for Pap Smear Cytology

The hospital where I work recently acquired a physician’s practice, and we are now sending out and billing for pap smears. For the LabCorp test we are using, the appropriate CPT/HCPCS codes would be 88175/G0145. My question is which code do we use? The CPT or HCPCS code? Are there different situations that would warrant use of one over the other (such as with Medicare patients)? Any help would be appreciated!

pap smear question

Hello,

This should be a simple question but it’s one that my coworkers don’t agree on.

If a patient with commercial insurance has an annual gyn exam and pap smear done in the office – besides 99394-99397 -, are there any other CPT codes that should be used? An insurance rep once told me to use G0124, which I don’t agree with. Also Q0091 has been thrown out there too. Any input would be appreciated.

Abnormal Pap Smear

How long can my provider continue to code abnormal pap smear after normal result? For example, patient had abnormal pap smear in 2013, repeated in 2014 and it was normal. Provider wants 5 years of normal results before she recommends having pap smear every 3 years. I’m not questioning the recommendation only how many years we can continue to use abnormal result diagnosis code. I cannot seem to find a clear answer on this. It seems to me that after the 1st two normal results we should be coding it as a screening with hx of abnormal pap if the provider wants it repeated yearly for this many years.

Medicare Pap Smear and office visit

We have a patient that does not want to have a full physical exam since Medicare doesn’t cover it and she is unable to pay out of pocket for the uncovered portion. Can we see the patient and bill an E/M code for the office visit portion to discuss lab results and then bill CPT Q0091 and G0101 for the pap, pelvic and breast exam? Will Medicare still pay for E/M? If so, do I use modifier 25 on the office visit? (we are her PCP)

Pap smear for medical products

Hi, is anyone there how to bill thew pap smears for medical and medical products as IEHP, vantage etc??? my Dr. was giving me an E/M code with Z01.419 and 88141 together.. which is completely wrong, I start billing 88142 with 88141 and mid 90 with Z01.419 for comercial products and so far it being paid, but not for medical .. how can i bill the papsmear for them??? im desperate!!!!!:confused:
