Hello, Our office is billing influenza A and B with Strep Tests on one claim for hundreds of kiddos. We use CPT 87804 QW, 87804 59,QW and 87880 QW with the ICD code for Fever. Our claims were being denied for not having a valid principal diagnosis. We switched to billing with J02.9 Acute Pharyngitis and J10.1 but these are denied for the same reason. Can anyone help me with this? I read online that we can’t bill both strep and flu together? Is that correct? Any help is appreciated!!

Rapid Strep with subsequent Strep Screen by Bacitracin
If an office does a rapid strep test (87081) and then subsequently performed a strep screen by bacitracin (87880), do we bill both of these? We have been billing both and I’m noticing some denials coming back saying that 87081 unbundles to 87880, but there is no CCI edit and I have been unable to find anything that addresses this or says we shouldn’t bill these together without a modifier. I think this is how this has normally been billed by all the offices but I am just now seeing some denials.

Flu and Strep test
I received my first denial for billing 87880 and 87804 (x2) together on a claim. This particular MCD payer denied the 87804 (x2) stating, "reimbursement included in another code per CMS/AMA/Medical guidelines.
I attached a different ICD10 to each test on the original claim. Has anyone else seen this denial and have you been successful in appealing it?
