Oral Fluid Drug Screen Testing

We are a CLIA approved lab that does in-house urine drug screens using cpt code 80305 with modifier QW. Recently our clinic started offering Vivitrol injections and part of the patients therapy is to be drug screened. It was decided to do the drug screens using oral fluid due to the swab being capable of detecting the presence of alcohol along with 13 other drug classes through optical observation. Some of the medical staff are concerned that this method isn’t CLIA approved and another would like to test for both the urine and the oral fluid; so can anyone tell me if there is a specific brand of swab we need to use or it doesn’t matter as long as it is coded once per day with 80305 – QW. Any input would be appreciated.

Rapid Strep with subsequent Strep Screen by Bacitracin

If an office does a rapid strep test (87081) and then subsequently performed a strep screen by bacitracin (87880), do we bill both of these? We have been billing both and I’m noticing some denials coming back saying that 87081 unbundles to 87880, but there is no CCI edit and I have been unable to find anything that addresses this or says we shouldn’t bill these together without a modifier. I think this is how this has normally been billed by all the offices but I am just now seeing some denials.

Extended drug screen labs PLEASE HELP

If anyone can help me or answer this question. We have a psychiatrist that has been ordering extended drug screen panels and she is marking it P1252 and that is not a code I know it has to be lab based not CPT based.

One I need to know what that code is used for if anyone knows and two I need to know if we are clia waived if this is a blood test we can do in house or if it has to be done in a lab setting.

Any answers are appreciated. My chief operating officer wants me to figure this out this morning so any help is welcome
