Aetna denying problem visits as part of global ob

We are having a problem with Idaho Aetna denying OB problem office visits as part of the global ob care, when clearly they are not. ie: URI, Headaches, skin rash, etc. Even after going through their "reconsideration" request (first level of appeal), with supporting documentation including CPT definition of ob global care and proof that we can charge separate office visits for unrelated problems, they are still denying it. It’s like they aren’t even reading my letters! We only get 2 appeals with Aetna, and this is a huge amount of money we are loosing. Now, they have started going back through visits they have already paid on, and taking their money back. I’ve audited all of these visits and they truly are reimbursable services.

I am at my wits end. Does anyone have any suggestions? Does anyone have this problem as well? We don’t have a provider relation rep assigned to us so I can’t complain to them, but seriously, Aetna can’t be withholding payments on billable services like this! HELP!

WHO SHOULD GET THE REFUND? Both Medicare and Aetna paid as primary

We have a patient in which both Medicare and Aetna paid for multiple dates of service as primary payers. Our office contacted both insurance companies who assured our office that they were in fact the primary payer.

In turn, we contacted the patient to ask that they contact Medicare and Aetna to update their coordination of benefits. The patient has since expired and we even called the patient’s spouse to ask them to contact the insurance companies regarding COB as well, with no success.

We are left with the question of who or how we should refund the overpayment(s) received. Any advice or input would be much appreciated.

CPT 81003 inclusvie denieal from Aetna.

Hi all,

Aetna insurance frequently denying CPT 81003 or 81002 charges as inclusive with E&M service (99201-99395).
Initially I tried with modifier “25” to E&M, after that I even tried with an appeal, but no use, it denied as inclusive again.

In this case I need clarification that, is there any payer policy in Aetna website regarding this.
Please help me with your opinions and provide me Aetna policies.
