Years of Experience before becoming an Auditor

Hi There –

I am curious, how many years of coding experience did you have before becoming an Auditor and passing the CPMA exam? I am currently a Coder and want to take the exam, however, I was wondering if it is possible to shadow someone already in the profession to see what a typical day looks like. I hear there is absolutely no coding involved. I don’t know if that is possible or not to shadow someone, but will look into it. :)

Industry Standards for Amount of Providers to 1 Auditor?

I’m looking for some information on what are the industry standards for how many providers 1 auditor should have? This would be based on a standard yearly audit for each provider, as well as fixing any errors that may come across our WQs as we call them. So far the consensus is, 1 Auditor: 50 Providers. Is there information somewhere on that?

Auditor question about 4×4 method for PE

Pennsylvania Medicare auditors score a physical exam as Detailed when there are 4 or more exam items noted in the medical record for 4 or more body areas or organ systems. They use the 1995 E/M examination guidelines utilizing this 4×4 tool. Do you think the following is sufficient documentation for Detailed PE (4×4 method) or should there be more details?:
"She is awake, alert and oriented. Lungs are clear. Heart exam reveals regular rate and rhythm. Abdomen is soft and protuberant with hypoactive bowel sounds, significant tenderness in the upper abdomen without rebound."

Help finding an auditor

Does anyone know if there are any physicians out there that do audits? We are looking for a new auditing company and they prefer an M.D. to be the one to do the audits.

Also…Does anyone use audit software in the physician office? We perform random audits but it is not via any software. We use an E&M audit template and go through each note selected.

Any advise helps. Thank you so much! :)

Auditor states drug ordered as IV over 2 minutes must have a doc stop time?

Hello everyone,

We are currently experiencing a situation in which an auditor is stating that all IV drugs ordered by the physician with an expected duration of 1-2 minutes must have a documented stop time in order to charge the 96374, 96375 or 96376.

I have never encountered a situation like this in over 5 years. Perhaps I’ve missed verbiage in the CPT coding book that states it must be present?

Any help would be appreciated.

Experience Revenue Cycle and Medical Chart Auditor

I have 25 years’ experience in Medical billing with advanced knowledge in Federally Qualified Health Centers, Private Practice Management and Rural Healthcare. Upon arrival at Operation Samahan, Inc I discovered the organization was still using the 2014 Medicare PPS rate, this was updated. Then rebuilt the billing department, created and sent statements and collection letters for the first time in the history of the organization. Also, the organization was in debt to the State of California for $ 8.5M, and additional amount of $ 2.5M to be added for the FY17, I improved billing and reconciliation so that for the FY17 reconciliation with the State of California came to approximately $ 150K. Corrected the uploading of file for a Capitated plan, as OSI was not receiving capitation payments. While for a short period of time I over saw the call center, I implemented automated call reminders. Created a sliding fee discount tracking, reduced AR days from 106 to 30 and retrained all front desk and MA’s on registration. Cleaned the system up to reflect true A/R

Auditor Productivity Standards

I am looking for any information anyone might have on auditing productivity standards. Not coding productivity but Auditing. Is there a standard on the amount of records that an auditor should be able to do per hour, per day or per week? We have standards of about 11 records per hour but I believe that might be affecting accuracy.
