We got B08.4 (Enteroviral vesicular stomatitis with exanthem), but my doctor says that this code seems to reflect mostly on the stomatitis, but not the actual condition of the disease. Any better coding for this (systemic virus infection with manifestation on hand, foot and mouth)

I do coding for a large physician group and after the physicians add their codes I review them for accuracy. I am having difficulty deciding when to remove symptoms that I believe are an inherent to a disease. For example: M47.892-Other spondylosis, cervical region with M54.2-Cervicalgia. I believe I would remove the cervicalgia but I am questioning myself. Does anyone have any advice as to what I can do to study this area of coding?

Our EMR puts past dx in the history and lets the provider activate it as needed. Our provider would deem its Hypertension and skip the HHD dx. Our coder comes back insisting that the provider click HHD. Our provider is in the opinion that she could code it from the past med hx.
