IUD Strings not found on exam

Hello. I am needing advice on what ICD 10 code would you use for an ultrasound that was done because the provider could not find IUD strings on exam. The IUD was in the correct location so we coded Z30.431 The insurance company is denying the claim saying she is not covered for routine ultrasounds. I re-sent the claim including documentation which they reviewed and still denied because of the dx code. I am unsure of another code to use.

ICD10 found on previous visit.

If a more specific diagnosis is found on a prior office visit, can it be billed?
For example, sleep related-leg cramps is listed on a January office note, but I’m working on Feb 16 of this year, and leg cramps only is documented.
If I look up a prior note and it says "sleep related leg cramps" can I bill this since it’s the more specific dx?
(I know the note being billed must stand alone, but on the other hand, I thought that if the final dx is known at the time of billing, it can be coded)

Acne Help Found At A Medical Spa

Your local medical spa offers a host of beauty treatments performed by trained professionals. Among other forms of treatment, their services extend to helping teens and adults afflicted with pimples and blemishes find clear skin once again.

Acne is often thought of as a skin problem which only affects adolescents, but this isn’t true. Many adults carry the malady past their teen years or even develop it during adulthood as a result of stress or hormonal issues.

There are various things that cause a flareup of acne. Some medications that dermatologists prescribe can be very effective but are fraught with serious risks of side effects. Some complications may include birth defects in a woman who is pregnant while ingesting the drug, menstrual irregularities, inability to sleep and severe depression. Many patients still give the prescription medications a try and carefully monitor the side effects with the help of their doctor.

At the medical spa, there are many ways to get rid of acne:

– Facials may help, which include special cleansers that address a person’s specific skin type. Astringents and facial tightening masks may help, as well. Aromatherapy is offered at many locations in order to add a comforting sensory dimension to this treatment.

– Microdermabrasion and Dermabrasion: Both microdermabrasion and dermabrasion offer skin re-surfacing help in order to remove the upper layer of dermis, leaving a smooth new layer when all heals. One method uses crystals as the planing agent while the other uses a rotary device.

– Chemical peels: This type of resurfacing treatment option is performed by applying chemicals to the upper surface of dermis in order to start fresh. There are various levels of peels, including deep, medium and light duty.

– Laser resurfacing: A laser can be used to remove a blemished and scarred upper layer of skin.

– Photodynamic Therapy (PDT) is another option. In this therapeutic treatment, blue light is aimed at the problematic region to kill bacteria. The bacteria is often part of the culprit causing the break outs in the first place. Photodynamic Therapy leaves the skin surrounding the treated region unaffected and healthy.

A patient comes in a Problem A (the "chief complaint"), but during the exam the provider also found problem B. Can problem B be added as a secondary chief complaint?

(The reason we want to do this is that our software only gives us an HPI checklist for chief complaints. If a problem is not a "chief complaint," the provider has to free-text the HPI; also the software will not count this HPI toward the E/M level.)
