Need advice-Stay at home mom with previous 10+ yrs Coding experience cannot find work

I would like advice on what to do in order to get back into medical coding. I have 10+ years of coding experience. I have been a stay at home mom due to my husband receiving military orders overseas from 2012-2015. I last coded in 2012 and since I do not have current ICD-10 experience, I cannot find work. I have taken and passed the ICD-10 exam but need to know what other steps I should take in order to get back into coding. I have been working from home doing HEDIS abstraction since 2016 but our contract is currently on hold and I would like to get back into medical coding. Any help is much appreciated. Thank you!

Coping and Pasting previous records.

I am coding for Oncology and my physicians are coping and pasting notes from previous records and putting them under History of Presence of Cancer at the beginning of the chart. When they take it from more than one previous record they do include month and year of the each record. Can I count this as review and summarization of old records? Any advise would be greatly appreciated.

ICD10 found on previous visit.

If a more specific diagnosis is found on a prior office visit, can it be billed?
For example, sleep related-leg cramps is listed on a January office note, but I’m working on Feb 16 of this year, and leg cramps only is documented.
If I look up a prior note and it says "sleep related leg cramps" can I bill this since it’s the more specific dx?
(I know the note being billed must stand alone, but on the other hand, I thought that if the final dx is known at the time of billing, it can be coded)

Supervision of pregnancy following previous miscarriage

How would you code the supervision of a 25 week pregnancy, for patient who has a history of a previous miscarriage? Would you code O09.292, O09.899? Or would you not go high risk at all for the pregnancy? Here is some information from the ultrasound report. I am being instructed that O09.292-supervision of pregnancy with poor reproductive or obstetric history is the correct code. But that seems more severe than what this patient’s history indicates so I wanted to get a second opinion from the group here as I dont code obgyn regularly.

INPATIENT: Chronic hypertension. Fetal anomaly suspected. Fetal anatomy survey.

History

OB History Gravida: 2. Para: 0.

New Year’s Coding Books – Transferring Annotations and Notes from previous year

How many of you fellow coders out there transfer your markups/highlights/notes, etc., from your previous year’s code books (ICD, CPT, HCPCS, etc.) to the new year’s code books? Is this something you routinely do every single year? How long have you been doing so? As a new coder, I did this from my 2016 to 2017 books and it took me 2 weeks! I wish there was a better way. How do the rest of you manage? Do you supplement with electronic resources in any way instead of marking up the paper books? Thanks in advance for sharing-!!
