Coding heart cath, w/ coronary angiography

**My first post**
I’m not a professional coder by any stretch but I’ve been trying to learn more since I came into the cardiology field. One of our docs did as follows:

Left heart catheterization with coronary angiography, left ventriculography, angioplasty, carotid arch and four vessels, abdominal aortogram, and right selective runoff. (Report available if more information is needed)

The coder billed 93458.26, 36200, and 75716.

Our reimbursement was only $ 278.98 and I feel sure that a modifier should be attached to not only the 36200, but also the 75716. Insurance denial just states that the 36200 & 75716 were included in the 93458.26. I was thinking the 36200 would need a mod 50, and the 75716 would need to have a 26 & 59. But again, I’m not a coder, just trying to learn as I come across denials so I can fix them.

Left and Right Heart Cath

Hi,

I have a report that has left heart catheterization and a right heart catheterization. Left heart catheterization was completed. When doing the right heart catheterization did not advance the Swan
Ganz catheter in the pulmonary wedge position due to the shape of the heart and frequent ventricular ectopy. Is the Right heart Catheterization billable?

Billing injection for supravalvular aortography without heart catheterization

How can I bill an injection for supravalvular aortography without doing a heart catheterization? It is for a PDA stent placement. I am also billing 76937, 36215 and 37236. Catheter placement in carotid artery… Any suggestions?

broken port a cath retrieval via heart cath

One of my cardiologists performed a heart cath (via the subclavian and the groin) in order to retrieve a broken piece of a port-a-cath that had floated into the atrium and then lodged in the ventricle. We have never had to code this procedure before; and I am looking at 37197. He states that he had go through both locations simultaneously to guide the piece out of the ventricle and out of the body. Any help would be greatly appreciated!

I am working as medical coder i have a doubt regarding cardiac catheterization. my question is what is the difference between left heart catheterization and right heart catheterization. when physician will go for left heart catheterization or right heart catheterization. can anyone clarify my doubt.

Hypertensive Heart Disease

My coder attended a seminar and was told that if patient has had Hypertensive Heart Disease (w or w/o heart failure) that it never goes away and should be in all the subsequent visits’ diagnosis and assessment list.

Our EMR puts past dx in the history and lets the provider activate it as needed. Our provider would deem its Hypertension and skip the HHD dx. Our coder comes back insisting that the provider click HHD. Our provider is in the opinion that she could code it from the past med hx.

Who is right?
