Documenting chronic health problems with a comprehensive Preventive medicine visit

I’m wondering if anyone can shed some light or provide direction for me regarding what can or should be addressed during a patients preventative medicine exam
I understand the difference between a Medicare AW exam and the age appropriate exam CPT codes 99381-99387 and 99391-99397
My query lies with the later the none Medicare exam
Components include appropriate history and exam with preventative counseling based on age as well as risk factor reduction,Immunizations and diagnostic’s
Where the water gets muddied for me is when the patient has long term chronic problems which are all brought into the encounter as well.
Is it necessary ? to discuss the pt’s COPD ,NIDDM , sarcoidosis etc during their preventative medicine visit ,assuming all conditions are stable at the time of the visit and if so to what extent
Understanding that if there is a significantly problem or abnormality found or discussed such as an exacerbation of the pt’s COPD that would be addressed with an E&M.
What I want to be sure of is it necessary to address all comorbidities during a preventative visit with a separate assessment and plan for each
Obviously the patients entire health picture is taken into consideration for counseling etc however the focus sometimes shifts from preventative a follow up visit on the chronic problems Not to mention the extra time spent addressing each co morbidity
If anyone can provide insight or recommend literature to research I would appreciate it
Is there a way to bill for extended Preventative visit when the provider addressed several stable chronic medical problems,as they can be quite lengthy

Diabetic Eye Exam in PCP setting (Internal Medicine)

I work in a Primary Care office and my physician is thinking about purchasing the machine/software to perform diabetic eye exams. What would be the correct CPT code since it’s being done in the primary care office setting? We’re needing to check on reimbursement so I want to be sure I have the correct codes.

I would really appreciate any feedback if any of you have experience with this! Thanks in advance!!

Hi everyone, does anyone out there have experience with MFM billing/coding. I’m finding alot of the insurance companies don’t like the dx codes given to me by the Dr. I’m also seeing their documentation doesn’t support the codes…any help or suggestions would be much appreciated!!!

Preventive Medicine Services & Office/Outpatient

I have an OB visit that was denied. A 99385 was billed for a new patient along with 99213-25 on the same day. The 2018 CPT book states on page 36 that if a problem or abnormality is significant enough to require additional work, then the appropriate Office/Outpatient code 99201-99215. Can 99203-25 be billed with the 99385 since it is the same date of service and it is a new patient? Or is only one "new patient" procedure code allowed? Insurance denied Z31.41 with the 99213-25 even though the additional time spent was for infertility testing.

Any help is greatly appreciated! :)

Hi there!!
I am an RN at an Occupation Medicine practice, in my past life I was a CPC (still certified) so I have been assisting with some auditing and billing questions. Does anyone out there work for an occ med practice that bills strictly worker’s comp or the employer? We are a fairly new practice. I am just looking for what others do for CPT, lead provider thinks time based billing is best but isn’t always applicable in my opinion. Just reaching out to see if anyone else has knowledge in occ med billing.

The art of tattooing is finding its way into the medical field. In medical procedures, making a permanent mark on the body is becoming a practice. Tattooing is used in the medical field for healing purposes and sometimes an unwanted tattoo is done on the awkward area; even without the patient’s consent.

What do you understand from the term “medical tattoo”?

This is not one of those trendy patterns or designs of tattoos that the doctors do on you in the hospital to satisfy your tattoo desire. But rather, it is a method of treating the illness. Yes, it is a tattoo but with a different set of goals and rules.

Medical tattooing is practiced mostly in the field of cancer therapy. It is done in the form of radiation. Radiation is the best method for treating few types of cancer. Since, the treatment for cancer goes on for a few months. The area to be treated is marked or tattooed. This is the best and reliable method of marking the affected and treated areas, so that the doctors do not have to remark or retrace the area again. This helps the oncologist to follow a proper track.

Tattooing is used for reconstructive purposes called the micropigmentation. In this medical field, aureola reconstruction and scar re-pigmentation are the most popular areas, where tattooing is done. This option is followed for treatment after post surgery. Replacing larger areas of skin due to skin pigmentation or burnt skin; has brought the doctors and the tattoo specialists together. They work along with the patient to restore back their looks or the skin that they have lost during the surgery. People can match the ink colors with their skin and thus filling up the places that has lost the color of the body. These inks come in different tones and shades that is blended and applied on the skin. This type of tattooing works better with people of darker skin color. This does not work with the Caucasians. White tattoo inks are available but it cannot be used on the skin having a dark tattoo color. A dermatologist would prefer using a laser treatment to remove the skin pigmentation in the above case.
