Where to find the link to get the paperwork to upgrade CPC-A to CPC-C on portal?

Hello,
I recently passed my exam this month. My professor has sent the paperwork that I qualified with the types of classes I had to upgrade it. Yesterday, I saw 2 resources are required one being from your employers. Today I am unable to find the link to get the forms for my employer to complete them. I would love assistance on anyone that can direct me back to where I need to be for this.

Patient Over Paperwork

Hey everyone :-)

I’ve been spending a lot of time reading about the new Medicare proposed rule…Patients Over Paperwork. My provider (Internal Medicine/PCP) is really concerned about the decrease in reimbursement and we’ve had conversations about needing to see more patients…which seems pretty impossible.

I would love to hear some thoughts from other offices and figured this was a great place to start.

CMS proposal; Patients before paperwork

So I am putting this out there because I feel that if it’s not being talked about(LOUDLY) then that is a problem. CMS has a new proposal that would almost eliminate the need for documentation completely for E/M visit, specifically NP or EP OVs. I’ve included a link to the CMS site so that anyone can go and get educated on this topic.
