Performed a repeat OB ultrasound due to the prior ultrasound finding a marginal placenta previa. Today’s ultrasound finds no placenta previa. Findings were left posterior placenta. I am unsure how to code this finding. Hoping someone can help me out.

Hi fellow coders
Please help with a coding issue. We have an attending physician that requested we get the placenta from the birth hospital for an infant being treated at our facility. The placenta arrived along with slides (we did not request the slides, just the placenta) and their report. We grossed the placenta and the pathologist will review the outside slides along with the slides we processed. Do I code this as an outside consult even though the outside facility did not request the consult (88323)or level V specimen (88307)? Thanks

Stillborn delivery of placenta only
I need help with a 20 weeks delivery, patient had delivered fetus prior to dr entering the room. Dr only delivered the placenta. I am questioning myself as to whether it should be 59414.

Delivery at home and placenta delivered at hospital
Patient is 28 with IUP at 39w4d who presented to labor and delivery via EMS after delivering at home. Our physician stayed on the phone with husband until EMS arrived. EMS delivered the baby and then brought the patient to the hospital. Placenta had not yet delivered. Our physician delivered the placenta. How would this be coded (CPT and DX)? We have not come across this situation before. We have been following patient for her OB care. Thank you advance for your responses.

Abruptio placenta is defined as the separation of the placenta from the wall structure of the uterus. This is a significant and perhaps life threatening problem observed in women normally in their final trimester of a pregnancy.

Symptoms – A woman suffering from this situation may encounter average to serious bleeding. Diagnosis – If the signs and symptoms talked about above are noticed the mom is in need of immediate analysis by a physician. The evaluation may consist of a physical examination, blood exams, ultrasound studies, or other specialized exams.
Remedy – Abruptio placenta can be a life threatening problem for both the mom and fetus if bleeding is extreme and right medical attention is delayed. With immediate and correct healthcare care, each mother and fetus may survive and recover completely. Hospitalization is virtually always essential and an obstetrician may recommend quick delivery via vaginal or cesarean section. If bleeding is excessive, bloodstream transfusions or intravenous fluids may be required. Aspirin must be avoided as it may increase the bleeding.
Abruptio placenta is a critical ailment that demands quick medical attention. The cause is not properly recognized but with prompt healthcare attention both mom and child can survive this ailment.

Pt presented to the ER w/ umbilical cord hanging from vagina, with severe blood loss. US did not show fetus, but showed retained placenta. Patient was taken to OR and due to dilation of the cervix provider was able to remove the placenta w/ his hand.
