ICD10 J section code also note

At the beginning of the J section in the ICD10-CM book, there is a note to code also, "Where applicable" the smoking status of a patient. I would like to know if anyone has a coding clinic answer as to whether this "where applicable" pertains to where the code also is noted in the tabular or if it pertains to where a smoking status is noted in a record? How are those in the ENT field interpreting this if there isn’t an available coding clinic answer?

ICD.10 for global c section high risk pregnancy

Hi eveyrone:

Long time coder but first time coding OBGyn. Not sure what dx to use for global c section for high risk pregnancy. Pt had severe pre-clampsia. Should I use as the primary
dx O09.893 with Z37.0 as second dx or should I use O34.211 as primary dx?

CPT code for Excision of Abd Wall Mass (Keloid Scar) s/p Cesarean Section & Hyst

Hi there, I am new to this forum :)
I am trying to get the best CPT code for the following description "Suspect cutaneous nerve entrapment in knot of keloid tissue s/p cesarean section and total laparoscopic hysterectomy, Pfannestiel incision; approximately size of scar tissue is 1 cm in left lateral edge. Will be an open approach for superficial wound exploration."
I’m thinking 11401 for wide local excision of cesarean scar but have used 49203 in the past. Any advise would be greatly appreciated. Thanks! Jenette

Cesarean Section with TAP Block later in the Day Separately Billable?

Hello, if a patient has a c-section (CPT 01961 anesthesia charge), and the anesthesiolgoist visits the patient later in the day and does a TAP block CPT 64488 due to much postop abdominal pain on PCA morphine so patient concents to TAP block.

Is the TAP Block CPT 64448 separately billable in this case with modifier 59? Any advice would be greatly appreciated, thank you!
