Multiple Procedure discounts, apply to fee schedule or discount later?

Good Afternoon,

Was wondering if anyone applies multiple procedure discounts to their fee schedule or if you just discounted later on?

I would post full charge, with multiple procedure modifier, and then discount after process if applicable.

Was wondering what others thoughts are on this. Thank You!

Cesarean Section with TAP Block later in the Day Separately Billable?

Hello, if a patient has a c-section (CPT 01961 anesthesia charge), and the anesthesiolgoist visits the patient later in the day and does a TAP block CPT 64488 due to much postop abdominal pain on PCA morphine so patient concents to TAP block.

Is the TAP Block CPT 64448 separately billable in this case with modifier 59? Any advice would be greatly appreciated, thank you!

Billing for pap only on a later date from “well woman” exam

Hello,

One of our providers has asked me a question about properly coding well woman visits when the pap, (for whatever reason) occurs on a later date, separate from the initial exam. Technically, the pap should be included in the well woman exam, however, since it is being performed on a later date, we are having trouble deciding how to code it so the patient does not get billed for an uncovered well woman exam because it would fall inside the same year, which is generally all that is allowed. I hope this makes sense. I also think that it will be payer specific, but if I can have a general answer to start with, that would help a lot. Please let me know if I need to clarify anything. Thanks!!
