Type 2MD Coding

Hi

I am working a practice that consistently codes E11.9 Type 2 Diabetes Mellitus without complications) with DM codes that include complications. My physicians are trying to convey that the blood sugar itself at the visit is controlled but there are still issues with the conditions associated with the DM. Can we code E11.9 and complication codes associated with TYPE 2 DM at the same time/on the same visit? Please help

Type 2 diabetes mellitus with hyperosmolar nonketotic hyperglycemia

am confused whether to use E11.69 Type 2 diabetes mellitus with other specified complication or E11.65 Type 2 diabetes mellitus with hyperglycemia; I cannot code this to E11.00 Type 2 diabetes mellitus with hyperosmolarity without nonketotic hyperglycemic-hyperosmolar coma (NKHHC) because coma is not documented.

Flap reconstruction for Gustillo Type IIIB/IIIC

My co-worker and I are having a rather large disagreement on how to code these procedures. We will have a patient come in with a Gustillo type IIIb or IIIC and ortho will do their thing and then we will provide the flap coverage to cover the open wounds. I say we use the fracture codes since they include the open wounds. My co-worker disagrees and wants to use unspecified open wound codes. Any thoughts on this and does anyone have any articles that will settle this once and for all? Unfortunately my system doesn’t allow me to cut and paste the op note and it’s a four page note so I can’t really type it all here but any help would be appreciated.

Diabetes Type 2 with Circulatory Complications – PLEASE CHIME IN!!

This a general question. In a case were the Dr. documents "Diabetes Type 2 with other circulatory complications" and the patient actively has CAD (which is a circulatory system issue) as well, but the Dr. does not specifically say that the CAD is the diabetic complication. Is it ok to accept the code of "Diabetes Type 2 with other circulatory complications" or should it be down coded to just "Diabetes Type 2" since the Dr. didn’t specifically state that the CAD was the complication?

A co-worker and I going over this issue. I stated that I would code it as "Diabetes Type 2 with other circulatory complications". He stated that he would down code it to "Diabetes Type 2" since the Dr. didn’t specifically note that the CAD was the circulatory complication.
