Auditing global prenatal visits

We provide global maternity care for OB patients and typically bill 0502F (non-Medicaid). Say for instance, the patient has pre-existing diabetes and lists it as a diagnosis on the prenatal flow sheet. Patient comes in for a routine prenatal visit (0502F) but doesn’t mention anything about the diabetes….can I still attach the pre-existing diabetes diagnosis on that visit even though it wasn’t addressed at that visit but was found on the flow sheet?

I get that a visit note must stand alone….but this is part of global OB care and that prenatal flow sheet is documenting all her antepartum care.

Billing for pre-op visits is a national trend ???

I have an admin who is pushing some of the coders to find a way to bill for "pre-op" visits. The admin is under the impression that there is a legitimate way to do so. I have not heard anything like this and would like to hear from anyone who has "found a legitimate way" to bill for pre-op visits so I can understand where the admin is coming from.

Personally, the pre-op visit is included with the AMA & CMS global surgical package, so this does not sound at all correct to me.

99211 nurse visits

Get this from my manager “nurses can charge 99211 for a nurse visit. They have been shown how to chart it so it’s just a nurse visit. If they are giving the injection then they charge the injection code and the medication”. Please advise!! I’ve never been paid for nurse visits in other practices where I’ve worked. Thanks

Need advice on telephone visits

Does anyone have any experience billing for telemedicine? We are a PCP office looking for a little advice on the topic. I imagine each payer is going to have it’s own guidelines in place for telephone visits. (And I’m sure Medicare doesn’t reimburse for it) What CPT codes do you use?

nurse visits???

I work at an Urgent Care in a town that has a shortage of medical providers. Often people will call their PCP and they can’t get in to see them for weeks and new patient appointments are an average of 3 months out. As a result we often get patients who come and check in and then once they are roomed they will tell the MA that they want to be seen for a chronic condition or something that is out of our range of care. We end up spending the time to call their clinic and intercede to try to get them in to see their PCP and then we check them out and don’t bill for anything. This happens a lot and I am wondering if anyone has any opinions/suggestions as to whether we could bill a nurses visit for this or if I should just let it go?

How to Bill 2 Mental Health Visits on same day for FQHC

Does anyone know how to Bill 2 Mental Health Visits on the same day for FQHC electronically? The codes I have are 90792(telemedicine visit) and 90837 and the G code of G0470 automatically populates with each one as it is added.

I have billed an E/M Office visit code and mental health code before but I cant get this one to work and it says that a modifier can’t be used. Any suggestions will be greatly appreciated! TIA!

Diet Visits – 99401

If we see a patient for a diet visit, is it appropriate to bill for the counseling code 99401-99404 if all criteria is met, or is this only billable with a wellness visit? One of our coders thinks this is may be considered an add-on code like the smoking cessation counseling (99406). Also, if our doctor gives the patient a Bumex injection (J1940) or a B-12 injection (J3420), is that billable with the counseling code 99401-99404 along with the administration code 96372?
