Medicare and Medicaid billing for an audiology office

Hello.

1. If I bill a procedure code that is covered and payed by my private payers, but it is a code that medicare and medicaid don’t cover, do I still bill medicare and medicaid, knowing full well they won’t cover?
2. Do I have Medicare and Medicaid patients sign an ABN form? And bill them for it?
3. Do I just not bill Medicare, Medicaid and the patient? Since I know its not a covered benefit.

Any information/guidance you could share would be most appreciated.

Thank you.

Audiology- same day billing

I work in an audiology outpatient clinic. I had two audiologist see the same patient on the same day. Audiologist “A” billed 92567 and the “B” billed 92555. The following week the same incident occurred but with a different patient. This time Audiologist “A” billed 92550 and “B” billed 92579. Is there any coding or billing guidelines stating or not that two audiologist can bill different diagnostic testing from the same location, with two different NPI’s , and on the same day of service?
