How to code diagnosis diabetes mellitus with A1C

I have had 2 different doctors in the past week list their patient’s elevated A1C with the diabetes diagnosis. Listed in the Assessment, the doctor wrote as the patient’s diagnosis "Diabetes Mellitus, Type II with A1C 8.7". For the 2nd doctor, he wrote the unforgivable…"Uncontrolled diabetes, A1C, 7.6". I understand that we cannot code from lab values but both doctors are clearly trying to express the patients have DM with hyperglycemia. In both H&Ps, the doctors discuss the patients needing to watch their diets, loose weight, etc, etc. So what should I code these as? E11.65 or E11.9?

hypertension due to diabetes

Hello- i’m trying without much luck to find any guidelines on how to code when a provider documents "hypertension due to diabetes" I know that DM and HTN do not have a causal relationship and there are no combination codes to link them together.

One resource (see below) I found says you would code the E11- DM dx with the I15.2 hypertension secondary to endocrine disorders as 2ndary- I have not been able to find any other supporting documentation for this coding suggestion.

adverse effects of corticosteroids on diabetes

Hello. I am confused on the sequencing of steriod-induced diabetes mellitus due to the prolonged use of corticosteroids. Subsequent visit for the diabetes. The drug was discontinued at the previous visit.
E09.9 and T38.0X5D

The answer I am being given is sequence the T38. code first followed by the E09 code.

However, ICD 10 states under E09 Drug or Chemical induced diabetes mellitus:

Use Additional

code for adverse effect, if applicable, to identify drug (T36-T50
with fifth or sixth character 5)

Code First

poisoning due to drug or toxin, if applicable (T36
-T65
with fifth or sixth character 1-4 or 6)

The answer given seems to go against the guidelines.
Is the diabetes a manifestation of the steroid use or an adverse effect?
If it gets sequenced second- why?

Diabetes and Hylercholesterolemia/ High Cholesterol

Having a issue with the E11.69 diabetes with specified complications. If the patient has diabetes and hypercholesterolemia, is the relationship assumed if not linked in documentation? I say no. If hyperlipidemia is not assumed, how can high cholesterol? Some are disagreeing with me. Please help.

Coding Diabetes with Hypertension

Looking for opinions on coding Diabetes with Hypertension.

When coding in book, hypertension is not listed as a specified complication. Since hypertension is considered a circulatory complication I feel the correct code is E1159 rather than the more unspecified code of E1169 (other specified complication).

How do you code this? Do you know of any Coding Clinics regarding this?

Diabetes Self Management Training

We are starting to perform Diabetes Self Management Training to our patients with HGB above 9. This is an effort to provide higher quality care to our patients and to satisfy MIPS measures. We have a medical assistant and a RN at our office. both of whom have been trained by Novo Nordisk Clinical Education program, which is certified by NCQA. We just started the one-on-one sessions with the patients today. The sessions include education on proper administration of insulin, nutrition, and developing an overall Diabetes Management plan individualized to each of those patients. Most of our patients are of Medicare age.
I am unsure how to bill for these services. 99211 for nurse visit? or 98960? Any guidance on this would be greatly appreciated.
